First Visit

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, treatment can be done on the same day as your consultation. However, a complex medical history or treatment plan would require an evaluation and a second appointment to provide treatment or diagnostic procedures on another day.
Please assist us by providing the following information at the time of your consultation:
- Your referral slip and any x-rays if applicable.
- A list of medications you are currently taking.
- If you have dental insurance, please bring the insured person’s information. This will save time and allow us to generate a claim form for you.
All patients under the age of 18 must be accompanied by a parent or guardian at the time of the consultation visit.
Radiographs
If your dentist has taken recent x-rays, you may request that they forward them to our office. For your convenience, digital x-rays can be emailed or mailed to our office. If there is not enough time, kindly make arrangements to have your x-rays picked up to bring them with you for your consultation. Should additional radiographs become necessary, they can be taken at our office.
